Spinal and Epidural Anesthesia

Epidural Information

An epidural and spinal are both put in through a needle in the woman's back.  The spinal cord is covered by a tough membrane that protects it and keeps the spinal fluid inside where i flows around the brain and spinal cord.  There are several layers.  The innermost is called the "dura."

An epidural is "above the dura."  So anesthetic is injected between the dura and the membrane above it in a "potential space."  The membranes will separate if fluid is injected between them.  The meds can transfer through the dura to the spinal fluid where they bathe the nerves and cause the "nerve block."

A spinal has the anesthetic injected into the spinal column by sticking the needle all the way through the membranes and directly into the spinal fluid. The the meds work directly on the nerves exposed to the fluid.  The level has to be high enough to block pain, but not block respiratory nerve centers.

Epidurals are harder to put in.  It takes experience and a careful touch not to pop through the dura.  If this happens, it is called a "wet-tap" and the epidural ten is really a spinal.  Spinals are usually a one-shot deal for the birth or for surgery.  Depending on the level, they can be called various things such as a "saddle-block" or "caudel," which is injected low into the area where a bundle of nerves comes off the spinal cord and numbs the area a saddle would contact and down.  After a spinal, depending ont he methods and policies, a patient has to lie flat on her ack for 8-12 hours in hopes of preventing a spinal headache, which is severe and lasts for days to a week. If a "wet tap" epidural occurs, the risk of a headache is pretty high.

Epidurals usually are put in to stay with a small plastic tube which is taped to the mom's back throughout the birth an can be used for a c-section and for all medication.  Medication can be injected intermittently or continuously via an infusion pump.  Medications vary, are usually a combination of "cain" anesthetics and narcotic analgesics.  They do get to the mom's bloodstream and do affect the baby.  An epidural can be spotty in how well it works.  Mom's may feel pain in some places and not in others. Afterwards, lots of women have trouble passing urine.

When the mom gets an epidural, she must have cardiac-respiratory monitoring, an IV and continuous fetal monitoring.  She gets a catheter as well.  Positioning is an issue because the epidural causes low blood pressure, which can affect the flow of oxygen to the baby.  Depending on how early in labor an epidural is started, contractions may slow or stop so then mom gets Pitocin as well which is the start of the intervention highway.

Effects on the infant can last a month or so.  Babies are usually not as alert, floppy, don't want to root or suck, which can have an effect on mom's self esteem.

Unfortunately, they don’t realize their labor starts at home and may continue for many hours before they go to the hospital.  Then they may need to stay in triage for quite a while as they are examined and wait for a room.  As their fear escalates, so does their discomfort so that many women are yelling and asking for an epidural before they are 3-4 centimeter, which is when an epidural is usually given. 

One of my HypnoBirthing moms was in triage and very comfortable at six centimeters while the women next to her was very upset.  My concerned HB mom was told that the woman was only one centimeter dilated.  The other woman was terribly afraid which caused her to create tension in the uterus which caused pain. I will show you how to circumvent this.

Epidurals can slow or stall labor and usually do.  When this happens, Pitocin is given to speed the labor up so that more epidural is needed.  This intervention can cause mom to spike a fever which can cause fetal distress.  This can lead to a C-section and is why most hospitals have over a 40% C-section rate.
